abendbrot Posted October 27, 2017 Report Share Posted October 27, 2017 Dear Potential Participant, My former student Melissa Kosky, and I are conducting a research study entitled, “Evaluation of young adults’ decision-making process in choosing whether to obtain a second cochlear implant”. We are collecting information about young adults’ demographic data, such as gender and age. There are also questions about your hearing loss and the hearing technology that your use or have used, as well as your perceived auditory abilities. Additional questions focus on the decision-making process that you have experienced, when considering a second cochlear implant device. The findings from this research study will provide professionals and young adults with information about the cochlear implant decision-making process. The requirements for study participation are: ● Between the ages of 18-35 years old ● Have one cochlear implant device received by the age of 18 years ● Have considered getting a second cochlear implant device after the age of 18 (regardless of whether or not you obtained a second cochlear implant device) Participation in this study is completely voluntary, and there is no compensation for participating. This questionnaire will take approximately 10-15 minutes of your time.
